Option 1 - Up to 3 Month Supply - Phase 1

No monthly visits are required. We are available by phone or email with questions and will check on you. Shipping is available for $25. All supplies are included in this price.

3-month supply when paid in full.

.25mg weeks 1-3

.50mg weeks 4-6

1mg weeks 7-9

1.5mg weeks 9-12

*titration to lower or higher dosing does not impact pricing. Treatment after phase 1 will be priced based upon the patient treatment plan

Option 2 - Up to 3 Month Supply - Phase 1

Once weekly visits with the nurse for medication and supplies. Supplies and medication will be secured at the office.

Months 1-3

$65+/week

.25mg weeks 1-3

.50mg weeks 4-6

1mg weeks 7-9

1.5mg weeks 9-12

.25mg-.75mg = $65/week

1mg-2mg = $85/week

2.5mg = $110/week

Missed visits will be charged an additional $50 and are at risk of treatment discharge.
